What Medicare really costs in 2026

Medicare isn’t free, and the costs change every year. Here’s every 2026 premium, deductible and copay in one place, plus a calculator for what you’d pay each month.

Part A · Hospital
$0

premium for most people

Part B · Medical
$202.90

standard monthly premium

Part D · Drugs
$2,100

yearly cap on drug costs

Medicare Advantage

Premium Calculator

What will you pay each month?

Your Part B and Part D premiums depend on your income from two years ago. Enter yours to see your 2026 numbers.

Your details
Your 2026 Medicare premiums, per person
Part A premium$0.00
Part B premium$202.90
Part D income surcharge$0.00
Each month$202.90
That's $2,434.80 a year, before any Medigap, Part D or Medicare Advantage plan premium.

A Real-World Example

A long hospital stay adds up fast

Say a fall leads to a 75-day hospital stay. With Original Medicare alone, you’d pay the $1,736 deductible plus $434 a day for days 61 through 75.

That’s $8,246 for one stay, before any doctor bills. A Medigap plan like Plan G pays all of it.

Part A in 2026 · what you pay
Hospital stay
Days 1–60$1,736 deductible
Days 61–90$434 a day
Days 91 and up$868 a day*
Skilled nursing facility
Days 1–20$0
Days 21–100$217 a day
Days 101 and upAll costs

*You get 60 lifetime reserve days in total. After those are used, you pay all costs. A new benefit period starts after you've been out of the hospital or facility for 60 days in a row.

Good to Know

What people ask us about Medicare costs

Premium higher than $202.90?

It's based on your income

Social Security uses the tax return from two years ago. Higher incomes pay an added amount on both Part B and Part D.

Retired or cut back on work?

You can ask for a lower bracket

If a life change like retirement lowered your income, file form SSA-44 with Social Security to have a newer year used.

Money tight?

Help paying is available

Medicare Savings Programs can pay Part B premiums, and Extra Help lowers drug costs for people with limited income.

Why do costs change each year?

Medicare resets in January

Premiums, deductibles and limits are updated every year. We update this page as soon as the new figures come out.

Still working at 65?

Your employer plan may change the math

Depending on your employer’s size, keeping work coverage and delaying Part B may cost less. We’ll compare both for you.

Which path costs less?

It depends on how much care you use

Medicare Advantage often costs less in a healthy year. Medigap usually costs less in a year with a big hospital bill.

Know your real costs before you choose a plan.

We’ll add up premiums, deductibles and drug costs for the plans that fit your doctors and prescriptions, so you can compare total cost, not just the monthly price. In-person or virtual.
